Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family.  We will request that you complete our new patient forms. This paperwork provides us with your health history and information on your condition. Our initial registration and history form is located in the forms section of this website if you choose to fill that out ahead of time.

Consultation

Next, you will have a consultation to discuss why you are here and what led up to your condition along with any other health problems you have. This initial visit is designed for Dr. Rogovin to learn more about you, your condition, and to determine how we can help you.


Examination

After your consultation, Dr. Rogovin will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.


X-Ray Studies

Depending on your specific condition, we may have X-rays taken as well. X-rays can alert us of  any serious spinal conditions if those are suspected or if trauma has occurred.  Many of our patients do not need X-rays and some do.


Report of Findings

Once the information is collected and examinations are performed, Dr. Rogovin will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?

After reviewing your health history, goals, and examining your spine, Dr. Rogovin will discuss recommendations and notify you if your condition  requires care with other providers.   Dr. Rogovin will  provide the best treatment and  wellness program for your needs.


Treatment

 At the conclusion of this initial appointment, you will experience your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage.  Treatment is interactive, so you can express concerns about the different styles of treatment  anytime.


Nutritional and natural pain relief

Prior to leaving, the doctor may suggest you check out our natural muscle relaxer: Formula 303. This is not a drug but consists of minerals and herbs that naturally relax tight muscles and calm the mind. Many patients take these nightly at bedtime for a more restful sleep.

We also have Biofreeze, a very good topical pain relieving gel.

We also may suggest Probiotics. What is a probiotic?

Probiotics are bacteria that help maintain the natural balance of organisms (microflora) in the intestines. The normal human digestive tract contains about 400 types of probiotic bacteria that reduce the growth of harmful bacteria and promote a healthy digestive system. The largest group of probiotic bacteria in the intestine is lactic acid bacteria, of which Lactobacillus acidophilus, found in yogurt, is the best known. Yeast is also a probiotic substance. Probiotics are also available as our dietary supplement.

What are probiotics used for?

In most circumstances, people use probiotics to prevent diarrhea caused by antibiotics. Antibiotics kill "good" (beneficial) bacteria along with the bacteria that cause illness. A decrease in beneficial bacteria may lead to diarrhea. Taking probiotic supplements (as capsules, powder, or liquid extract) may help replace the lost beneficial bacteria and thus help prevent diarrhea.

A decrease in beneficial bacteria may also lead to development of other infections, such as vaginal yeast and urinary tract infections, and symptoms such as diarrhea from intestinal illnesses.

Research has shown that certain probiotics may restore normal bowel function and may help reduce:1

  • Diarrhea that is a side effect of antibiotics.
  • Certain types of infectious diarrhea.
  • Inflammation of the ileal pouch (pouchitis) that may occur in people who have had surgery to remove the colon.

These results suggest that eventually probiotics may also be used to:

  • Help with other causes of diarrhea.
  • Help prevent infections in the digestive tract.
  • Help control immune response (inflammation), as in inflammatory bowel disease (IBD).

Researchers are studying the use of probiotics for inflammatory bowel disease, colon cancer, and irritable bowel syndrome (IBS).

Are probiotics safe?

Probiotic bacteria are already part of the normal digestive system and are considered safe.

 

The doctor may also discuss with you ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.   Every person is unique, therefore everyone requires a  customized plan.  The purpose of our wellness program is for you to achieve good spinal alignment, have a  healthy diet, exercise, and maintain a positive mental state.
